Medication Authorization Forms
A Fulton County Schools Medication Authorization Form is required for each type of medication (prescribed and /or over-the-counter) a TMSA student is required to be administered by the school nurse. In some cases, the Care Plan Form, completed by the student's doctor/pediatrician, is also required. For example, if a student has asthma, both the Medication Authorization Form and the Care Plan Form are required. For the administration of only over-the-counter medication, the Medication Authorization Form is required. Contact TMSA's School Nurse if you have any questions or concerns. Click here to access and obtain the forms.
Student Code of Conduct and Discipline Handbook
All TMSA parents, guardians and students may click here to view and review the 2023 - 2024 Fulton County Schools Student Code of Conduct and Discipline Handbook. After reviewing the handbook, all TMSA students above 10 years of age and their parents, guardians, or other person having control or charge of the child are asked to digitally sign acknowledging receipt of the Code of Conduct and Student Handbook.
Join the PTO
Join The Main Street Academy’s Parent Teacher Organization (PTO). Membership for the 2023 – 2024 school year is $15 per household and proceeds support teachers and events, such as Teacher Appreciation Week.
PTO hybrid (in-person and virtual) meetings will be held on the first Tuesday of each month during the 2023 - 2024 school year. The first PTO meeting of the 2023 - 2024 school year will be hosted on Tuesday, September 12, 2023 at 4:30 p.m. inside the school's Media Center and virtually.
Also, support the PTO by purchasing official TMSA spirit wear (children and adult sizes are available). Click here for more details.
Support the Annual Innovation Fund
The Annual Innovation Fund is TMSA's premier fundraiser and the most important annual giving opportunity at our school. All supporters of the TMSA community are encouraged to participate and make a tax-deductible donation to the Annual Innovation Fund between July 1 - June 30 each school year. We're seeking 100% participation from TMSA families to help achieve the 2023 - 2024 Annual Innovation Fund's goal of $100,000. Every TMSA household is asked to contribute a minimum of $100 annually to the Annual Innovation Fund. However, no contribution is too big or too small. Click here for more details and to make your household's annual contribution online.
