Medication Authorization

TMSA parent/guardians must complete Fulton County Schools Medical Authorization Form(s) for medication to be administered to and/or carried by students during school hours. Click here to access the forms and return them to Nurse Jackson. 

 

Over-the-counter (OTC) medications require a parent/guardian signature only.

 

Prescribed medications require both parent/guardian and physician signatures. 

 

Nurse Jackson cannot administer medications that are not provided by parents/guardians and accompanied by the appropriate paperwork.

 

Medications must be brought to school in-person.
