A Fulton County Schools Medication Authorization Form is required for each type of medication (prescribed and /or over-the-counter) a TMSA student is required to be administered by the school nurse. In some cases, the Care Plan Form, completed by the student's doctor/pediatrician, is also required. For example, if a student has asthma, both the Medication Authorization Form and the Care Plan Form are required.
