TMSA parent/guardians must complete the Medical Authorization Form(s) for medication to be administered to and/or carried by students during school hours.

Important Reminders:
- All medications must be new, in box, unexpired, and age-appropriate for over-the-counter medications.
- All prescription medications MUST HAVE PHARMACY label attached.
- All forms must have a parent signature.
- All prescription medications must have both a provider and a parent/guardian signature.

Nurse Jackson cannot administer medications that are not provided by parents/guardians and are not accompanied by the appropriate paperwork.
- Medications must be brought to school in person.
